چکیده

Methods We included 20 mechanically ventilated patients. Respiratory system compliance was calculated by dividing the volume change induced by small changes in pressure-support level by the corresponding change in mean inspiratory airway pressure with subsequent adjustment for the pressure-support termination criterion (25% of peak inspiratory flow). To determine the respiratory time constant (τ), every expiratory flow-volume curve was divided into ten slices of equal volume. τ was calculated as the mean value of the curve ́s slopes in every slice of one breath only in the breaths with linear flow-volume relationship. Respiratory system resistance was derived by dividing compliance by τ. As a reference, we determined respiratory system mechanics measured during volumecontrolled ventilation with constant inspiratory flow.

UPDATE IN INTENSIVE CARE MEDICINE: MECHANICAL VENTILATION New modes of assisted mechanical ventilation

Recent major advances in mechanical ventilation have resulted in new exciting modes of assisted ventilation. Compared to traditional ventilation modes such as assisted-controlled ventilation or pressure support ventilation, these new modes offer a number of physiological advantages derived from the improved patient control over the ventilator. By implementing advanced closed-loop control system...

Visualisation of Time-Variant Respiratory System Elastance in ARDS Models.

Model-based mechanical ventilation (MV) can be used to characterise patient-specific condition and response to MV. This paper presents a novel method to visualise respiratory mechanics during MV of patients suffering from acute respiratory distress syndrome. The single compartment lung model is extended to monitor time-varying respiratory system elastance within each breathing cycle. Monitoring...

High-frequency oscillatory ventilation in neonatal RDS: initial volume optimization and respiratory mechanics.

To determine whether initial lung volume optimization influences respiratory mechanics, which could indicate the achievement of optimal volume, we studied 17 premature infants with respiratory distress syndrome (RDS) assisted by high-frequency oscillatory ventilation. The continuous distending pressure (CDP) was increased stepwise from 6-8 cmH2O up to optimal CDP (OCDP), i.e., that allowing goo...

Time-Varying Respiratory Elastance for Spontaneously Breathing Patients

Respiratory mechanics models can be used to optimise patient-specific mechanical ventilation (MV), but are limited to fully sedated MV patients who are not spontaneously breathing. This research presents a non-invasive model-based method to determine respiratory mechanics of spontaneously breathing MV patients. Patient-specific respiratory mechanics of 22 spontaneously breathing patients are de...

Expiratory peak flow and respiratory system resistance in mechanically ventilated patients undergoing two different forms of manually assisted cough.

OBJECTIVE Mechanical ventilation is associated with retained airway secretions. Manually assisted cough contributes to the displacement of bronchial mucus, whereas positive end-expiratory pressure increases collateral ventilation and maintains airway patency. This study aimed to assess the effects of manually assisted cough, either alone or added to increased positive end-expiratory pressure an...
